Multiple drop boxes to control corresponding text boxes with same choices

hi,

 

im driving myself crazy trying figure out.

 

i have form has multiple (15) drop boxes in 1 column.  each drop box has 2 corresponding text boxes in same row/line.  instance, first line has drop box list of names.  next on same line 2 text boxes (address , phone #) populate depending on name picked.  continues down page drop box , 2 text boxes on each line.  every drop box has same list of names.  able first line name chosen populates text boxes, when add line 2, conflict name chosen in dropbox in line 1 begins control text boxes on line 2.  each line should operate independently of 1 another...

 

any ideas??

are field names (both of drop-downs , text fields) unique? code using, , placed?
