First survey form - would like quidance/suggestions

hello,

i have prepared survey in word.docx , wondering if expert can @ , make suggestions design. obvious, drop down list. if answers no , want them jump question, how do that?

if want start docx need convert pdf form first before importing formscentral. won't able use skip logic on form.
