is there way include "how did find us?" question , associated dropdown box on order registration page?

hi, try

in admin panel, go extended crm fields-> add crm form

in details, give name, in form field, add field name "how did hear us", click save fields. shown in image below

click next, save.

now go web form apply this,

while in form, go crm forms, select new crm form have created, shown in image below

click update.

now put web form on web page , publish page,
